I cant kill the 1st clown guy. I hit the speakers, then the lava comes up. He then turns into a big monster. I shot the monster heaps but he didnt die. He just killed me in 1 shot...

I did madness mode too!
To get the true face of evil badge, at the end instead of shooting him punch him. His mask falls of and you get the badge. Also you can then shoot him.
